docs: document completion verdict and bump version to 1.3.10

CHANGELOG 1.3.10 (unreleased) with behaviour-change and breaking
sections, README capability rows and a 1.3.10 section, ARCHITECTURE
section 15 plus 6.2/7 updates, schema doc for the stream_end column,
migration impact notes, ROADMAP entry, .env.example MISSING_DONE
semantics, and a revision pointer on the 1.3.9 design. Follow-ups
filed as #32 and #33.

## 1.3.10 影响核对(2026-10-14,#30/#31)
装配处 `retryable_exceptions=(TransientError, AllSourcesExhausted)`(§4)**不含** `ResultInvalidError`:文本 length/过滤且无正文改抛它后,AgentLoop 步级重试**不再**重抽该步——这正是本次修复的意图(确定性结局重抽只花钱)。业务侧若想对 length 截断做"加大 max_tokens 再试",须显式 except `ResultInvalidError` 并按异常消息原因码 `length_without_content` 分支;其余缺 `[DONE]` 而完整的流(#31)从此直接成功。
