iomgaa b4812e12c8 fix: degrade terminal telemetry failures instead of masking domain errors
终态出口 `emit_terminal_once` 此前只让 `_record` 内的 except 兜住落库,而
诊断字段的提取(`_error_fields` → `_structured_detail` → `format_bounded_errors`)
在降级 try **之外**求值。下游经公共端口(自实现 `StructuredOutputStrategy`
或 transport)构造出 `ResultInvalidError(validation_errors=(非 str,))` 时,提取期
抛的 `TypeError` 会顶替调用方本该收到的领域异常——错误四分类被击穿(下游
`except ResultInvalidError` 落空),且 `claim_terminal()` 已消耗故终态行照样丢,
同时违反"遥测写失败降级不冒泡"。

改法与 RetryMW 的 attempt 出口(`retry.py::_emit`)同款: 把快照冻结与 await
整段包进 try,`CancelledError` 原样上抛、其余落一条 warning。终态行按已批准的
best effort(兜底命中时该次逻辑调用 0 条终态行,不补写)。异常类型校验与
`ResultInvalidError` 的既有设计均未改动。

顺带同步审查报告的 Minor 项: SQLite recorder docstring 26 → 36 字段、
research-wiki 索引重建、ARCHITECTURE 的 `sampling` 段落终态调用点口径,
并删除 `TelemetryMW` 迁移后无读取点的 `self._now` 死字段(保留形参,
避免平白打断既有装配写法)。

iomgaa 393f2bf617 feat: record call observability columns and terminal failure rows
Grow the telemetry contract from 26 to 36 fields and give every logical
call a failure terminal row, so SQL can finally answer "how many calls
failed" and "why did the whole pool die".

Schema and port move together with the emitter writes in one commit:
splitting them would ship columns that nothing populates.

- schema: append 10 nullable columns (scope, operation, logical_call_id,
  event_kind, http_status_code, error_type, cause_type, error_body,
  attempts, total_latency_ms) to all five definition sites in one order
- ports: 10 keyword-only parameters without defaults; the protocol
  signature is now the single source the assembly gate derives from
- emitter: take domain exception objects instead of pre-flattened text
  and pin down the diagnostics in one helper; a relabelled 503 stays
  503 and success rows leave all five columns NULL
- emitter: reject recorders whose record_llm_call cannot accept the
  current field shape at assembly time, since _record would otherwise
  swallow the TypeError and drop every row while calls keep succeeding
- clients: write at most one terminal row per logical call through a
  single shared exit, deduplicated by the call context; TelemetryMW
  stops writing terminals so the two sites cannot double count
- clients: cancellation stays best effort and propagates, non-domain
  exceptions get no terminal row and keep their classification
- transports: give _status_to_error an explicit operation and fix the
  historically mislabelled embedding HTTP failures
- structured: promote the bounded error formatter so the reask feedback
  and the terminal explanation share one set of limits

Terminal rows carry no cost and no tokens, so cost aggregation is
unchanged; failure counts must now filter on event_kind.

iomgaa e06cd8e8b7 feat: record which tier a call actually ran at
Twenty-five columns and not one of them answered "which tier was this?",
so the question the whole issue exists to settle - does a higher tier buy
anything - had no way to group its data.

The three emit entry points deliberately disagree, the way sampling
already does. A successful attempt records what the transport actually
sent: with EFFORT_FALLBACK=nearest a request for medium goes out as low,
and recomputing here would file the row under a tier that never left the
process. A failed attempt has no response to read, so it falls back to
the requested tier - which is exactly right for the tier errors that are
rejected before any HTTP happens, because the rejected tier is the
signal. Cache hits and terminal failures have no chosen source at all,
so a source-level tier is not a thing they could report.

emit_attempt now demands to be told whether the path reasons at all.
Embedding and OCR share the emitter but never send reasoning parameters;
without the flag a source that mistakenly carries ENABLE_THINKING would
hang a tier on a call that could not possibly have run at one.

The value lands as a plain str. StrEnum is a str subclass and asyncpg
promises nothing about encoding subclasses, and a telemetry write that
fails is only a warning - Postgres would just quietly lose the column.
NULL means nobody declared a tier, which is not the same statement as
'none', and the two must never be folded together.

iomgaa 1307a02b92 fix: close the failure modes review found in the new code
Three of them were the same shape as the bug this branch exists to fix:
something goes wrong, the library swallows it, and the caller is left
with a number that means the opposite of what happened.

The throttle key had no source in it. Five sources on one model is the
normal case here, so the first one to break would warn once and silence
the other four for the life of the process, and the message never said
which gateway to look at.

An unknown verdict in a cached entry threw away the whole response. The
rehydrator tolerates unknown fields but not unknown values of a known
field, so two library versions sharing a Redis would each invalidate
the other's entries: halved hit rate, and the only log line says the
cache rebuild failed. A purely observational field should not be able
to void a response whose content is intact.

Normalising for telemetry now degrades instead of raising, both for a
bare string and for a value outside the domain. Either one used to
reach the same except and cost the whole row, which is exactly how
1.3.0 lost nineteen calls without anyone noticing.

iomgaa 56acb8f3ac feat: record the reasoning verdict in telemetry
This issue surfaced only because someone ran a slow suite that is
excluded by default and had not been run for eighteen days. As a column
it becomes a query: which model stopped being observable, and when.

The emitter unwraps the enum to a plain str at the single _record exit.
asyncpg makes no promise about encoding a str subclass, and a telemetry
write that fails is downgraded to one warning — it would not crash, it
would just quietly cost the Postgres path a column. Normalising at the
emitter follows what tenant_id, meta and sampling already do.

The column is appended last in COLUMNS and in both DDLs. An existing
table can only take ALTER at the end, so putting it anywhere else
forks the physical column order between a freshly built database and a
backfilled one.

iomgaa c26b34e854 feat: wire the telemetry text cap through settings
`PGW_TELEMETRY_TEXT_CAP` now reaches the emitter on every assembly path.
Unset means no truncation, which stays the default: a truncated row is
no longer audit evidence and cannot be replayed, and downstreams rely on
that today. The flip side — contracts and bids sitting in `llm_calls`
indefinitely, multi-tenant — is spelled out in `.env.example` so readers
can weigh both.

All three `from_settings` paths are wired (chat, embedding, OCR): they
write the same table, so capping only chat would leave half of it
uncontrolled. `TelemetryEmitter.__init__` now rejects `text_cap <= 0`;
it is the single point where the three clients converge, so the direct
construction path — a public assembly route the settings guard never
sees — is covered too. `0` would otherwise reduce every body to a bare
elision marker.

iomgaa 33ed7ecdfc feat: cap telemetry bodies at a configurable length
Chat rows stored full message and response text with no upper bound, so
downstream contracts and tenders lived in llm_calls indefinitely. Add
_cap_text/_cap_messages in the single telemetry exit (_record), applied
after digest_messages and before json.dumps, plus to response/thinking.

Capping is per text, not over the serialized JSON: cutting the whole
string would emit invalid JSON into an unvalidated TEXT column. The cap
builds new dicts and never mutates in place — digest_messages passes
non-list content straight through as the same object, so an in-place cut
would silently poison the caller's messages and the cache key.

text_cap is required on TelemetryEmitter (internal class, three known
construction sites) and defaults to None on the three public clients, so
the default behaviour stays byte-for-byte identical. Settings wiring
lands separately.

iomgaa 9bdd312928 fix: check the meta key budget before scanning every key
The key-count cap exists to catch a whole request body dumped into meta.
That is exactly the shape where the per-key regex runs tens of thousands
of times before the real reason surfaces, so the cheap check goes first.

Also correct two stale docstrings: postgres.py still claimed 22 columns
(it is 24), and _canonical_meta_json promised to raise on non-finite
floats. It is evaluated inside _record's degradation try, so the real
outcome is a warning plus a dropped row -- never an error the caller
sees. What the gate actually buys us is the SQLite side, whose meta is a
TEXT column that would happily store a literal NaN.

iomgaa dba706b59c feat: record each call's tenant and caller-defined dimensions
Both telemetry backends gain tenant_id and meta at the end of the
column list, and TelemetryEmitter fills them from the request. The two
halves ship together because the emitter is the only caller of
record_llm_call: adding the columns without filling them leaves every
row short of two keys, and the backends read those keys outside their
try block, so the KeyError degrades to a warning and the whole table
stops filling.

The columns are appended, never inserted. An old table can only gain
columns through ALTER, which puts them last; a new table built from the
DDL would put them wherever the DDL says. Anywhere but the end and the
two paths produce different physical column orders, while the INSERT
uses positional placeholders.

The two backends spell the default differently for different reasons.
SQLite refuses a NOT NULL column without a non-NULL constant default
outright, so the default is what makes the backfill legal at all. On
Postgres a non-volatile constant default is what keeps the ALTER from
rewriting the table, and NOT NULL DEFAULT '' is what keeps old rows out
of the black hole a NULL tenant_id falls into under an RLS policy.

Normalisation happens in the emitter, not the recorder, matching how
canonical_sampling_json already settles the sampling column: None
becomes the empty string, an empty mapping becomes the literal '{}'.
Keys are sorted so one set of dimensions serialises identically on
every row, and allow_nan=False is a second gate behind the entry
validation -- json.dumps would otherwise write a bare NaN, which JSONB
rejects, and the failed insert would be swallowed as a warning.

All three emit entry points read the request. Cache hits read it too,
rather than the replayed response: the dimensions answer who made this
call, not who made the one whose result is being replayed.

iomgaa a57a5cea72 fix: let assembly defects pierce the gate wrappers
Independent verification caught that the split shipped in the previous
commit did not actually hold on the only path production uses. The gate
wrappers re-raise GovernanceBackendError but nothing else, so
SourceNotConfiguredError fell into the following `except Exception` and
came back out as a governance_backend_down failure with retry_after_s=5.0.
A misconfigured source name would still retry forever and never surface.

The existing tests missed it because both of them call the private _cfg()
directly, one layer below the wrapper the governance loops actually go
through. The regression test goes through QuotaGate.

telemetry.py has to widen its terminal catch in the same commit: once the
wrapper stops relabeling the error, it is no longer a GovernanceBackendError,
and it is raised before any attempt exists, so the path would have recorded
no telemetry at all.

Also corrects the leak path count from three to five. QuotaGate.stats and
BreakerGate.retry_after_s are not wrapped by _record_quietly either.

iomgaa b1109e9fe9 docs: fold the Codex review into the issue #7 design and register it
Pins the ARCHITECTURE section 6.1 revision to land before or with the
implementation, since the new scope reason contradicts the current single
source of truth. Documents why SourceNotConfiguredError may sit outside
the four-way classification: that rule governs transport-translated call
failures, and the GatewayUnavailableError family already lives outside it.

Also collapses the ten per-field response ternaries in emit_attempt into
an _AttemptUsage view. They all expressed the same decision and pushed the
method to cyclomatic complexity C, which blocked the commit gate.

iomgaa 89ff916bc8 feat: collect reasoning_tokens from the provider usage payload (issue #6)
Reasoning tokens are already counted inside completion_tokens, so the
cost total was never wrong -- what was missing is the attribution: how
much of a call was spent thinking rather than answering.

LLMResponse and TransportResult each gain a trailing reasoning_tokens
field, and the telemetry port grows from 21 to 22 columns with the new
column appended in both backends so fresh and migrated schemas keep the
same physical order.

None means this particular call did not report the field, not that the
source never reports it: a relay that falls back to a local tokenizer
replaces the whole usage object and drops completion_tokens_details.
Downstream checks must therefore read "in (None, 0)"; no provider was
observed reporting a literal zero.

iomgaa 42e429eb58 fix: void the cost of rows whose usage is unavailable
失败尝试与终态失败行的 usage_source 由 estimated 改 unavailable(用量确实
不可得),并在 TelemetryEmitter 的成本换算里为 unavailable 短路记 NULL。
短路刻意插在 cache_hit 分支之后: 缓存命中未产生新调用,0.0 是事实而非未知。
附 OCR 成功行的防回归钉(仍为 measured、settle 恒 0,设计 §3.3 剔出决定)。
